Restorative care—an option for our seniors recovering from illness or injury

Older adults, whether residing in independent living or in an assisted living or personal care home, may face health care situations requiring a temporary change in their senior living status. That’s when restorative care, like Pathways Restorative ServicesTM at Country Meadows Retirement Communities, becomes a valuable option for senior community residents.

Short-term and long-term care
Our Pathways service offers short- and long-term assistance to those needing more support than can be provided in our personal care setting. Someone recovering from surgery or injury, or who has been discharged from hospitalization or rehabilitation, may benefit from restorative therapy to help them get stronger. Our service is designed, as well, for seniors with a chronic condition or illness, such as joint replacement or stiffness, arthritis, chronic pain, muscle weakness or incontinence.

With Pathways restorative care, we have developed a goal-focused, action-oriented program, customized to the abilities of each individual. In contrast to some nursing homes or other care settings that foster physical dependency, Pathways is based on residents’ existing skills and their direct involvement in their care.

Pathways program helps dreams come true
With determination and a positive outlook, Country Meadows residents can use Pathways to make dreams come true. Ann, a resident at York-South (Leader Heights), set a goal and made a memory she’ll always treasure. Through diligence and dedication to her goal, she was able to gain strength and walk down the aisle at her granddaughter’s wedding. “It was one of the most satisfying moments of the day,” says her son Tom. Though Ann did the hard work, she gives the credit to the Pathways program.

Focus on individual abilities and goals
Our program incorporates seniors’ goals and aspirations for a better quality of life into the foundation of their plan. Our restorative care team, committed to empowering residents to use their abilities to achieve independent senior living, personalizes their plans with steps to reach their goals. These therapeutic plans may include:
• Physical, occupational and/or speech therapy
• Fitness programs
• Emotional and spiritual support
• Nutritional services
• Specialized approaches to accomplish daily tasks (bathing, dressing, walking and more)

By helping seniors increase strength and energy, reduce disability and lessen their risk of falling, restorative care allows them to live more independently, healthily and safely. It can even provide them with a more positive outlook and improve their mental well-being.
