As part of our commitment to protecting coworker and resident information, George M. Leader Family Corp. requires vendors and business associates with an active Business Associate Agreement (BAA) to complete a Third-Party Security Questionnaire.
The questionnaire helps us confirm that appropriate safeguards are in place to protect protected health information (PHI) and meet HIPAA and our security requirements.
Vendors, contractors or business associates that create, receive, maintain or transmit PHI on behalf of Country Meadows and have an active BAA.
