After hospitalization, Progressions offers short-term restorative senior care

In a recent blog post on restorative senior care, we noted that many older persons aren’t physically ready to go home after hospitalization or rehab. While a temporary stay at a nursing home may be in order, increasingly, many families prefer the alternative of restorative care in a more home-like environment.

Now, Country Meadows Retirement Communities has a new restorative senior care service to help area seniors achieve their recovery goals and prevent re-hospitalization. Our Progressions program helps them increase strength and energy to live more independently, more healthily and more safely.

Progressions is an ideal solution for anyone who needs a respite stay for four to eight weeks—and costs far less than a nursing home. But Progressions, an extension of our Pathways Restorative Services™, entails much more than rest and relaxation. As its name implies, its intent is to help people make physical progress so they can resume a more active, enjoyable lifestyle.

Personalized therapy goals and plans guide seniors toward recovery

Progressions is designed for people to be active in their own recovery. Our restorative care professionals work with each participant to create an individual therapy plan listing goals and how to achieve them. The focus for each person is to improve skills needed to perform daily tasks, while receiving personalized senior care and hospitality from our support team.

This program would benefit any older adult who needs:

  • Significant assistance with activities of daily living (ADLs) and mobility (including a two-person or mechanical lift transfer)
  • Seven days a week of therapy and restorative support around the clock
    • 24/7 access to nurses
    • Support with co-morbidities and chronic diseases (daily weights, skin checks, blood pressure monitoring)
    • Special dietary, medication (Coumadin®, insulin) or wound care support
    • Coordination of care resources
    • A vibrant, home-like environment to foster the social and emotional aspects of healing

Our Progressions program also offers these senior care services:

  • Access to an on-site certified therapy gym and assistive devices
  • Medication management
  • Daily therapeutic activities
  • Assistance with bathing, dressing, hygiene and laundry

Participants in Progressions also receive three chef-prepared daily meals, personalized dietary support, a comfortably furnished private apartment, a personal bathroom and transportation to medical appointments.

After their short-term stay in Progressions, seniors either return to their homes or decide to remain at Country Meadows permanently at a level of care appropriate for them.

    Progressions sounds exactly like what I will need post surgery for hip-replacement in February. But how do I ensure it will be available when I need it?
