What is Post-Acute Care?

When a loved one is recovering from a serious illness, surgery, or hospital stay, the journey to full health often doesn’t end when they leave the hospital. This is where post-acute care comes into play. Post-acute care provides continued medical and rehabilitative support to help patients regain their strength, manage their conditions and achieve the best possible quality of life.

Post-acute care encompasses a range of services that support patients in their recovery after a hospitalization. It bridges the gap between the hospital and home, offering a transition phase that ensures patients are not only physically recovering but also gaining the skills and confidence to return to their daily lives. The goal is to prevent rehospitalization, manage chronic conditions and help patients reach their maximum potential.

Types of post-acute care

Post-acute care can take many forms, including:

  • Inpatient rehabilitation: Intensive therapy in a specialized facility to help patients recover from major surgeries, strokes or severe injuries.
  • Skilled nursing facilities: Provide 24-hour medical care and rehabilitative services for patients who need more support than they can get at home.
  • Home health care: Medical and rehabilitative services delivered in the patient’s home, ideal for those who are well enough to return home but still need some level of medical support.
  • Long-term acute care hospitals: For patients with serious medical conditions that require extended hospital stays.

Comprehensive rehabilitation services

Our center offers a broad spectrum of rehabilitation services for individuals of all ages including physical, occupational and speech therapy. Skilled therapists work closely with each patient to develop personalized treatment plans aimed at restoring mobility, improving communication skills and enhancing daily living activities. As an alternative to hospital-based rehabilitation care, our short-term rehabilitation programs address conditions from knee or hip replacements to post-traumatic injuries to recovering from a serious health condition such as a stroke. Provided by our partner, Powerback Rehabilitation, therapies are offered in our onsite therapy gym up to six days a week, two hours a day, from Monday through Saturday.

Skilled nursing care

Country Meadows co-workers in our Nursing & Rehabilitation Center focus on the whole person, ensuring individual physical, emotional and spiritual needs are met. Nursing professionals provide 24/7 skilled nursing long-term care to manage complex medical needs and administer medications, monitor vital signs and offer wound care, among other medical services. This ensures that patients receive the continuous care they need in post-acute care.

Personalized care plans

We understand that each patient’s recovery journey is unique. Our multidisciplinary team, including our medical director, nurses, therapists and social workers, collaborate to create individualized care plans. These plans address not only the physical aspects of recovery but also the emotional and psychological needs of each patient.

Supportive environment

Designed to provide a comfortable and supportive environment for care and recovery, our Center offers private and semi-private rooms, nutritious meals and recreational activities to promote social interaction and mental well-being. Our goal is to make our patients feel at home while they recover.

Family involvement

We believe that family involvement is crucial to the recovery process in post-acute care. We encourage family members to participate in care planning and provide education and support to help them understand their loved one’s needs. This collaborative approach ensures a smoother transition from our Center to home.
