Our Connections Program is offered in a variety of settings, including our personal care and assisted living communities, in secure neighborhoods and in our skilled nursing centers.
Level 1: The Connections Club
Residents in our communities who experience mild cognitive losses may be candidates for our Connections Club. This program was designed to alleviate feelings of frustration and anxiety that accompany early stage memory loss.
The staff in our facilities proactively identify these gradual changes and intervene so our residents can continue to experience success in their current environment. A program manager works with each resident member individually to develop a personalized plan for support. Using this plan, our club manager acts as a personal coach to help each resident member through the challenges and joys of daily living.

Monthly family support group meetings are also offered as one of the Connections Club programs.
Level 2: Connections Neighborhood
Seniors with more serious memory loss or who require the structure of a supervised setting or safety of a secured living area benefit greatly from our Connections Neighborhood. In the secured living areas our Connections Neighborhood is special because it offers secure entrances and exits while including access to an enclosed garden area and walking path.
Residents in the Connections Neighborhood receive all of the programs offered in the Connections Club, but with added support with daily activities, hygiene, continence management and personal care. Residents also receive more extensive therapeutic programming including opportunities for purposeful activities that help reduce anxiety and frustration.
Level 3: Connections Skilled Nursing
At our Bethlehem and South Hills of Pittsburgh communities, the nursing center also offers a Connections Neighborhood with security. These areas offer programming as described above, but also provides skilled nursing assistance for residents with more challenging medical issues.
