Seniors recovering from surgery or illness have options beyond nursing homes

Following surgery or illness, older adults aren’t always ready to immediately return to their homes. Many of them relocate to nursing homes, at least temporarily. But often, restorative care services can just as reliably—and less expensively—ease their transition from hospital or rehab to home.

Research studies indicate that seniors who receive restorative care after hospitalization experienced improved function over those who don’t. Restorative care has also been shown to lower their risk of readmission and emergency department visits.

At Country Meadows Retirement Communities, our restorative care program allows seniors to recover in a homelike setting rather than in skilled nursing facilities, often called nursing homes.

Our Pathways Restorative Services™ provides short- and long-term care

Pathways Restorative Services™ at Country Meadows offers seniors more support, both short-term and long-term, than they would receive in traditional personal care or assisted living. Pathways residents are generally recovering from surgery or an injury, or they might be living with a chronic medical condition.

Our staff of health professionals in Pathways share a philosophy of focusing on individual abilities and personalized goals. With customized plans for each individual, these experienced co-workers are dedicated to helping seniors increase their physical activity and independence.

Plans designed for seniors enrolled in Pathways at all Country Meadows campuses may include:

  • Physical, occupational or speech therapy
  • Fitness programs to improve strength, balance and flexibility
  • Emotional and spiritual support
  • Dining services to aid in overall nutrition
  • Specialized approaches to accomplish daily tasks

Therapeutic activities, a vital part of the Pathways program, help participants gain strength and energy to accomplish daily activities such as bathing, dressing and walking.

Our Pathways health care teams assist seniors in their recovery at all our campuses. Our locations are  in AllentownBethlehemHersheyMechanicsburg, South Hills of PittsburghWyomissingYork-SouthYork-West and in Frederick, MD.
